Facilitate home community care by engaging clients in social activities and encouraging participation in their interests. Uphold resident rights by respecting privacy and maintaining confidentiality at all times. Collaborate with healthcare providers to implement care plans tailored to individual needs. Maintain a clean and safe environment for clients, ensuring hygiene standards are met. Document care provided and any observations regarding client health or behavior. Qualifications Experience working with individuals with disabilities or in a group home setting is preferred. Background in nursing or home health care is a plus.
