Provide patient monitoring to ensure safety and well-being, reporting any changes in condition to healthcare professionals. Support individuals with mobility challenges, including transferring them safely from beds or chairs. Facilitate home community care by engaging clients in social activities and encouraging participation in their interests. Uphold resident rights by respecting privacy and maintaining confidentiality at all times. Collaborate with healthcare providers to implement care plans tailored to individual needs. Maintain a clean and safe environment for clients, ensuring hygiene standards are met. Document care provided and any observations regarding client health or behavior. Qualifications Experience working with individuals with disabilities or in a group home setting is preferred. Background in nursing or home health care is a plus. Strong skills in patient care and monitoring, demonstrating empathy and patience. Ability to cook nutritious meals that cater to dietary restrictions as needed. Excellent communication skills to effectively interact with clients, families, and healthcare professionals. Knowledge of resident rights and commitment to advocating for client needs.
