Care Coordinator Visiting Angels Brentwood, TN•weekends !!! 783 Old Hickory Boulevard, Brentwood, TN 37027 Overview Visiting Angels Nashville is seeking a dependable and highly organized Scheduler to join our team in Brentwood, TN. This key role ensures smooth coordination between clients and caregivers by managing daily schedules, urgent shift needs, new client additions, and first-time visits. Responsibilities include verifying caregiver shifts, tracking time-off requests, promptly addressing clock-in issues, and communicating schedule changes to clients within 15 minutes. The Scheduler will also conduct regular client follow-ups, review care plans with caregivers, and help manage caregiver preparedness. Additional duties include answering phones, maintaining accurate documentation, overseeing client and caregiver birthdays and anniversaries, handling angel badges and nominating caregiver of the month. This role is ideal for someone who thrives in a fast-paced, mission-driven environment and is passionate about helping others age in place with dignity. Duties Coordinate daily caregiver schedules, including urgent open shifts, new client assignments, and ongoing coverage needs Conduct initial visits with clients and caregivers to ensure comfort, understanding, and continuity of care Verify caregiver clock-ins and address any missed punches within 15 minutes of shift start Promptly communicate any caregiver or schedule changes to clients within 15 minutes of notification Follow up with clients and families after the first week of service and every 30 days to ensure satisfaction and quality of care Review care plans with caregivers prior to any first-time or fill-in assignments to ensure preparedness and alignment with client needs Manage and track all time-off and vacation requests from caregivers Perform general administrative tasks including data entry, filing, and maintaining office supply inventory Maintain accurate and timely documentation of schedules, communications, and client/caregiver interactions Answer incoming calls and provide excellent customer service to clients, families, and staff Oversee management of Angel Badges Track and celebrate caregiver and client birthdays and anniversaries Nominate and coordinate recognition for Caregiver of the

Month Qualifications Scheduling/Staffing Coordination:

Prior experience in scheduling or staffing, especially in home care, healthcare, or service-based industries, where managing coverage and quick response times are critical.

Customer Service:

A background in customer or client-facing roles helps ensure compassionate, professional communication with clients, caregivers, and families.

Home Care or Healthcare Industry:

Familiarity with the home care or healthcare environment, including understanding client care plans and caregiver responsibilities, is a big plus.

Crisis Management/Problem Solving:

Experience handling time-sensitive issues, such as last-minute call-outs or shift changes, and being able to resolve them calmly and efficiently.

Communication & Interpersonal Skills:

Strong verbal and written communication skills are crucial for following up with clients and coordinating with team members.

Technical Skills:
