Shared Living Provider – Addison County 3.5 3.5 out of 5 stars Middlebury, VT 05753 Pride Support Services is seeking a home provider who has a mother-in-law style apartment or similar setup to support a vibrant 68-year-old woman. She is independent with all activities of daily living (ADLs) but requires support with medication management. She thrives on being active and engaged, and enjoys shopping, swimming, fishing, gardening, and participating in her local community. She has two beloved indoor cats. They are well-trained, up to date on all vaccinations, and a meaningful part of her life. She is seeking a warm, welcoming, and supportive environment where she can maintain her independence while having the oversight needed to ensure continued success and well-being. A setting that allows her to continue living in the community, while providing the supports that foster her stability and engagement. would be ideal. Applicants must have a minimum of a high school diploma or GED, have a registered and insured vehicle, and a clean and valid driver’s license.
