We are a local non-profit organization serving children & adults with developmental disabilities. We are looking to fill a Full-time, salaried position, to provide case management and coordination for individuals with developmental disabilities, including intellectual disabilities, many of whom receive Medicaid-funded waiver services. Activities include planning, linking, and service coordination as well as monitoring of service provision. Position requires ability to organize, manage, and document complex services involving consumers, families, and private providers. Candidates need the ability to work effectively with a varied range of ages and disability levels. Requires knowledge of developmental disabilities and documentation procedures; and a working knowledge of client rights and local service systems. Bachelor’s degree in human services plus one year of experience working directly with persons with intellectual disabilities or other developmental disabilities is required.
