Job Description
Case Manager- DD Services
This position will be based out of our Houston County Day Treatment in Dothan, Al.
Monday-Friday 8:00am-5:00pm
Responsibilities:
Position provides case management services to consumers and families of SpectraCare’s Developmental Disability program
Process referrals for services of individuals within the catchment area
Develop written service plans and implement plans accordingly
Conduct periodic assessments and reevaluate service plans
Close out cases as appropriate
Requirements:
Bachelors degree in Psychology, Social Work or other human services related field
Must have a valid driver’s license with good MVR free from any reckless driving/DUI convictions for the past (3) years
Must be willing to submit to a pre-employment background check and drug screen
Must be willing to complete Alabama Medicaid Case Management training upon hire
