Full Job Description Overview:

The North Alabama Coalition for the Homeless is seeking a dedicated and organized Coordinated Entry Case Manager to join our team. This role focuses on assisting individuals and families experiencing homelessness by completing assessments, helping with identification needs, and connecting clients to housing and other community services. The position is based at our Client Center and primarily serves walk-in clients.

Key Responsibilities:

Meet with individuals and families experiencing homelessness on a walk-in basis. Complete Coordinated Entry assessments and input data into the HMIS system. Assist clients with obtaining documentation, such as photo ID and DMV records. Provide referrals to housing programs, healthcare, employment support, and other services. Build and maintain positive relationships with clients while supporting their progress. Collaborate with community partners to stay informed about available resources. Keep accurate and timely documentation in accordance with agency standards.

Qualifications:

Bachelor’s degree in social work, psychology, human services, or a related field (or equivalent experience). Experience working with individuals experiencing homelessness or vulnerable populations preferred. Familiarity with HMIS and Coordinated Entry systems is a plus. Strong communication, organization, and problem-solving skills. Ability to work independently in a fast-paced, walk-in setting.
