Overview Knollwood Healthcare is seeking a dedicated and compassionate Case Manager/Social Worker to join our team. The ideal candidate will play a crucial role in supporting individuals and families by providing guidance, resources, and advocacy. This position requires a strong understanding of behavioral health, crisis intervention, and social work principles to effectively assist clients in navigating their challenges and achieving their goals. Responsibilities Conduct comprehensive assessments to identify client needs and develop personalized service plans. Provide ongoing support through motivational interviewing techniques to empower clients in their recovery journey. Facilitate access to community resources, including addiction counseling, workforce development programs, and housing assistance such as Section 8. Implement crisis management strategies to address immediate client needs and ensure safety. Collaborate with multidisciplinary teams to coordinate care and services for clients, including those involved in older adults’ welfare and geriatric systems. Maintain accurate documentation of client interactions, progress notes, and service outcomes. Advocate for clients’ rights and needs within various systems, ensuring they receive appropriate support. Requirements Bachelor’s degree in Social Work, Psychology, or a related field; relevant experience may be considered in lieu of degree. Experience in geriatric health /behavioral health or mental health counseling is preferred. Strong skills in crisis intervention and management are essential. Familiarity with nursing homes is preferred Excellent communication skills with the ability to engage effectively with diverse populations. Knowledge of motivational interviewing techniques and workforce development practices is desirable.
