Provideresources and support to individuals with intellectual anddevelopmental disabilities, their families and guardians in orderfor the individual to be included in society, live as independentlyas possible and function at their maximum potential.

Job Responsibilities:

Follow all policiesand procedures set forth by the company and Alabama Department ofMental Health, Division of Developmental Disabilities, Office ofAutism Services, and Home and Community Based Services (HCBS)rules. Performs evaluations and assessments to meet the needs ofindividuals served Assists individuals served with applying forfinancial assistance, residential planning, vocational,recreational and educational desires, healthcare, in home supports,day services, legal, nutrition, transportation, social, and otherrelated services and resources. Advocate for services that willsupport the individual’s success Maintains all case recordsPrepares, updates and monitors person centered plans includingutilization of discovery and Person Centered Thinking (PCT) ToolsProvide support to individuals so they can participate in anddirect the person centered plan development process. Coordinatemeetings with individual, families and guardians, and members ofthe interdisciplinary team as required Establishes and maintainspositive relationships with individual, families, guardians, stateofficials and team members Records work and billing time inaccordance with company policies Job requirements: At a minimumpossess an bachelor’s degree in human services, social work,sociology, psychology, gerontology or least one year of relevantexperience in the human services’ field Hold a valid driver’slicense Maintain auto insurance Minimum of one-year experience inworking with individuals with disabilities, families and/orplanning and arranging services Travel up to 75%
