Job Summary:
Friendship Mission, Inc. is seeking a compassionate and highly motivated Case Manager to provide comprehensive wraparound case management and social services to clients residing at Friendship Mission. The successful candidate will play a pivotal role in assisting clients to break the cycle of homelessness by rapidly obtaining permanent housing and accessing necessary stabilization services. This role involves significant client interaction, community outreach, and meticulous documentation, all aimed at fostering self-sufficiency and stable housing outcomes.
Key Responsibilities:
Assess clients’ immediate life stabilization and housing needs, developing individualized plans focused on progressive transition to stable housing. Meet with clients weekly to track progress, discuss service referrals, and follow up on case/housing plan implementation. Educate clients on available local, state, and federal resources, facilitating referrals to promote stabilization and self-sufficiency. Document client progress towards housing plan goals in the electronic records system, adhering to agency standards and timeframes. Establish and maintain collaborative relationships with landlords, real estate agencies, and rental communities to secure permanent housing for clients. Interface with other staff and agencies to expand the agency’s resource database, initiate referrals, and ensure clients progress towards stable housing. Efficiently manage an independent caseload of 5-40 clients, supporting transitions to stable housing within a year or specified timeframe. Facilitate onsite workshops focused on life skills, health improvement, and interpersonal relationships to foster clients’ ability to obtain and maintain stable living conditions. Participate in multidisciplinary team meetings, in-service trainings, and other required gatherings. Comply with HIPAA Privacy Rules and FMI’s internal confidentiality policy, communicating pertinent clinical information with necessary client consents. Generate activity reports in a timely manner and participate in the collection of clinical outcome data as directed. Maintain assigned client records in accordance with program policies and procedures.
Minimum Qualifications:
Education/Experience:
Bachelor’s degree in Social Work or a related field with at least two (2) years of related experience. Years of experience may be considered in lieu of formal education where appropriate.
Skills:
Proficient in Microsoft Office. Excellent oral, written, and interpersonal communication skills. Knowledge of community resources is preferred.
Requirements:
Must possess a valid driver’s license and insurance.
Work Environment:
The Case Manager works in various settings of FMI, including offices, classrooms, and group rooms, as well as being involved in other areas of FMI programs both on and off campus. The position is also active in the community as situations warrant, representing Friendship Mission, Inc.
