CASE MANAGER JOB SUMMARY
Provide case management services to victim survivors of domestic violence/sexual assault who are residents at SafeHouse consistent with ACADV, ACAR, and agency standards Ensure quality and consistent case management services in emergency shelter Participate as a residential team member, assisting in coverage as needed Participate in the case management team/staffing Serve as a member of the on-call rotation team Share regular on-call duties after-hours and on weekends
ACCOUNTABILITY
Works under the general direction of and is accountable to the Residential Services Director and carries out responsibilities in accordance with SafeHouse agency standards.
QUALIFICATIONS
Education/Experience Bachelor’s degree in Social Work or related field and at least 2 years’ experience in direct service, prefer license or license eligible Demonstrated leadership ability and previous supervisory experience Bilingual in Spanish/English preferred Flexible schedule Computer (Microsoft Office Programs a must) Valid Alabama Driver’s License (or obtained within 30 days of hire date), automobile, automobile insurance and willingness to use said automobile in performing work duties Interpersonal Communications Demonstrates positive interpersonal communication skills to negotiate plans, dates and follow-up activities. Able to make presentations to the general public when called upon.
RESPONSIBILITIES
Provide case management services to survivors of domestic violence/sexual assault who are residents at SafeHouse consistent with ACADV, ACAR and agency standards. Provide initial intake in a timely manner consistent with agency policy Provide residents with relevant education, support and advocacy Work with residents to evaluate needs, write, and implement the case plan that is relevant to their individual needs Work to develop and document community resources to meet residents’ needs Answer crisis line calls and provide crisis counseling to victims Provide and/or coordinate referrals, support, transportation and other services as needed or requested Maintain strong awareness of clients’ rights and the importance of self-determination and empowerment Maintain a current resource log of resources within the community Coordinate weekly support and/or education groups for adult clients Assist with program, job, education and housing referrals for clients Serve as liaison with community service agencies for residents’ needs Works closely with community case manager in transitioning residents into independent living. Work effectively as part of a team Assist other programs when needed and does not interfere with the performance of his/her job duties Support SafeHouse outreach activities, including fundraising and support events Maintain a positive attitude and working relationship within all SafeHouse programs Participate in on-call rotation Model positive interpersonal relations with other clients, staff and colleagues Record Keeping and Documentation Keep records according to SafeHouse policy Maintain statistics for reporting to governing bodies as required Prepare monthly reports as requested Enter data into approved data bases as directed and in a timely manner
