Case Manager Position Available In Lincoln, Arkansas

Case Manager allheart Senior Care of Arkansas 610 S Lincoln Ave, Star City, AR 71667 Looking for a highly organized, motivated, and caring LPN or Social Worker to assist our clients with their supplies and needs. Someone who is able to take thorough and detailed chart notes, monitor and assess per Medicaid guidelines, can plan their day efficiently, and have genuine concern for the well being of their clients. Our company has 5 office to serve central and south Arkansas and provide non-medical, in-home assessments. This position requires driving to many locations. Company car and gas card are provided. Two (2) years of Case Management is required. Medical Specialty Case Management Responsibilities Comprehensive patient assessments Patient advocacy Maintain HIPPA compliance Coordinate managed care services Following POC set by DHS RN Referrals for needed DME supplies Referrals for needed services (legal, energy, housing, etc.) Assist in maintaining program eligibility Monthly and quarterly monitoring of patients Assisting patients with appointment scheduling Thorough documenting TCM billing for a minimum of 60 units per week Maintaining TCM Tickler System Conduct face-to-face and telephone contacts Collaborate with agency team Summary As an LPN/Targeted Case Manager, you will be integral in delivering comprehensive (non-medical) patient care and managing their needs. This role requires expertise in managed care and patient assessment. Your critical thinking and knowledge of medical terminology will be essential in collaborating with healthcare teams and advocating for patients. With a focus on both core and premium skills, you will contribute significantly to enhancing patient outcomes and overall service quality in our organization.
