Summary of Position:

The Continuum Services Counselor will be the primary source of supportive services to residents of Greenridge Place. This person will strive to ensure the well-being of Greenridge Place residents by coordinating services and providing residential support consistent with the philosophy of Masonicare’s Mission, Vision and Values.

Essential Duties and Responsibilities:

Strives to assure the highest quality of life and services for the residents. Establish support groups for all residents. Coordinate move-ins with various departments to ensure a smooth transition for incoming residents to the campus. Act as primary source of supportive services to new residents upon move in and provides an avenue for new residents to feel comfortable. Meet with resident and/or family within 72 hours and then determine follow up timeline Attains and/or maintains ongoing compliance with appropriate regulatory standards. Makes an active contribution toward improving community relations, increasing positive public regard and overall awareness of Greenridge Place. Actively participates in community relations initiatives to assure continued community awareness on the benefits the communities and resources throughout the continuum. Maintains confidentiality. Reports to, consults with, and informs the Executive Director regarding activities, needs and issues related to the residents of Greenridge Place. Works with residents and families regarding transitions of care of residency as needed. Provides assistance in crisis situation when conflicts arise with service providers, family members, and others. Responsible for assistance to residents related to Long Term Care billing, financial reviews, Subsidy Forms, Annual insurance reviews, and IL Annual Tax deduction letters. Attends and assists with educational programs and seminars to remain compliant with updates and standards of best practices. Provides follow up calls upon resident return from a hospital and rehab stay to welcome residents back to the communities. Attends meetings, provide resident support groups, serves on committees, and performs other special projects or tasks as assigned. Provides any additional support to the residents and staff.

Minimum Qualifications:
Education:

Bachelor’s Degree in Social Work preferred.

Experience:

1-3 years dealing with Senior Citizens in a professional capacity. Day Shift / 40hrs/wk
