Senior Outreach Case Manager
Wanted:
Versatile individual to join our team at Hands On Hartford as the Senior Outreach Case Manager with Hands On Hartford (HOH). The Senior Outreach Case Manager will play a lead role in engaging with individuals who are experiencing homelessness, in accessing the Coordinated Entry System and connecting with those who are not currently receiving case management supports, assessing individuals’ needs related to housing in order to assist individuals and families in finding appropriate, safe, affordable housing opportunities, assisting guests with meeting basic needs, client advocacy and ensuring individuals are successfully linked to resources as quickly as possible, providing referrals to and support guests in linking with both formal and informal supports.
As a Supportive Outreach Case Manager, you will:
Assist with the daily tasks of the homeless outreach program and will coordinate and collaborate with agency partners, government agencies and landlord/property owners around this project
Be responsible for smartsheets record keeping, HMIS, and participating in all assigned meetings to ensure the effective delivery of a myriad of homeless prevention services and resources
be part of the team that canvasses during non-traditional business hours; they will engage and provide services to individuals experiencing homelessness
Qualifications:
Ability to provide services in a culturally competent manner and ensure trauma sensitive care.
Fluency in and Spanish is a plus.
3 years’ of housing case management experience with demonstrated increased responsibilities required, experience with street outreach, strongly preferred.
The ideal candidate will have proficiency in Microsoft Word, Excel, Outlook and other Microsoft products.
Strong written and verbal communication skills.
Valid Driver’s license with insured vehicle required.
Ability to lift and carry up to 25 pounds.
When:
Full time; schedule consists of early mornings, evenings and weekends.
Pay:
Range $22-$26 per hour. This is an hourly position
