What You’ll Do As a Care Manager , you are the heartbeat of CHS care delivery. You’ll coordinate in-home support for clients, supervise caregivers in the field, and actively recruit, mentor, and retain an exceptional caregiver team. You’ll juggle client needs, scheduling demands, and staffing goals—all while keeping the human touch at the center.

Key Responsibilities Include:

Client & Case Management Perform biweekly/monthly home visits (scheduled and unannounced) to monitor client well-being. Develop and update individual care plans tailored to client preferences. Conduct initial assessments and maintain detailed case notes. Ensure services meet non-medical home care standards and provide client satisfaction . Coordinate with families, referral sources, and CHS team members to keep everyone informed. Manage accurate daily schedules and confirm visit coverage. Caregiver Supervision & Mentorship Provide hands-on training and coaching during on-site visits. Complete monthly performance check-ins and offer real-time feedback. Ensure compliance with care logs, protocols, and client safety procedures. Mentor staff on dementia and Alzheimer’s best practices. Recruitment & Retention Own the recruitment pipeline for non-medical caregivers. Post jobs, screen resumes, conduct interviews, and oversee hiring steps. Coordinate orientation and training for all new hires. Help create a supportive, team-first culture that keeps caregivers engaged and valued. Compliance & Documentation Maintain compliance with CHCPE standards and Medicaid guidelines. Monitor caregiver documentation (care logs, incident reports, change of condition). Ensure readiness for audits and quality checks.

What You Bring:

Associate’s or Bachelor’s degree in healthcare, human services, or related field (or equivalent experience). Minimum 3 years’ experience in home care case management, caregiver supervision, or client advocacy. Strong scheduling skills and experience with geriatric populations. Comfort working with dementia/Alzheimer’s clients. Bilingual English/Spanish is strongly preferred. Knowledge of Connecticut’s

CHCPE , DSS

, and local Area Agencies on Aging (SWCAA, AOASCC, WCAAA). Tech-savvy with Microsoft Office, iPads, and scheduling platforms. A valid driver’s license, clean driving record, and ability to travel up to 100 miles/day as needed.

Physical & Travel Requirements:

Ability to bend, climb stairs, sit/stand for long periods, and lift up to 30 lbs. Willingness to travel daily across Fairfield and New Haven counties using company vehicle (or personal vehicle as needed). Comfort navigating a variety of home environments and weather conditions.
