(RNP) , is a private, non-profit, social service agency which has been serving the Greater Bridgeport Community and Fairfield County area since 1972. RNP is dedicated to helping those living with substance use, mental health disorders, and homelessness. Our mission is to restore hope, health, and wellbeing for individuals and families in a recovery environment that embraces compassion, dignity and respect. Prospect House This program provides emergency shelter to thirty-six adults daily in a facility located in Bridgeport, Connecticut. The program provides comprehensive case management services to meet the varied and complex needs of individuals who are homeless, and work to connect clients to a long-term housing solution. The case management program at Prospect House includes assessment, transportation, life skills training, as well as individual, group, and family sessions.

Hours:

Monday•Friday, 9:00 am•5:00 pm

SPECIFIC DUTIES AND RESPONSIBILITIES

: All duties are subject to accommodation in accordance with the Americans with Disabilities Act (ADA). Assesses and records medical, psychological, and social factors contributing to the client’s situation and evaluates these capacities. Assertively links individuals to community resources and other natural supports. Develops and documents person-centered recovery plans. Maintain timely progress notes that record and monitor client progress regarding finances, employment, nutrition, basic needs, housing, physical health, mental health, and substance abuse-related issues. Serve as an advocate for clients in obtaining necessary services. Adheres to all standards as they relate to documentation requirements. Attend and be prepared for case management and staff meetings. Prepare a weekly supervision agenda. If appropriate, assume on-call responsibilities. Maintain a work schedule that allows for monthly weekend rotations. Assumes all other appropriate duties as deemed necessary by the supervisor.

QUALIFICATIONS

: Knowledge of principles and techniques of social service case management services. Knowledge of the physical, psychological, social and economic effects of substance abuse, homelessness, physical and emotional illness, crisis intervention and

HIV/AIDS

support. Ability to gather, analyze and evaluate information pertinent to client care. Knowledge of community resources. Considerable knowledge of federal and state laws and regulations regarding client confidentiality. Ability to establish and maintain cooperative professional relationships. Excellent communication skills. Ability to project a professional image.

EDUCATION AND/OR EXPERIENCE

: Bachelor’s degree in the behavioral sciences and two (2) years’ experience in counseling preferred, or equivalent combination of education and experience. Co-occurring competencies and or credentials are preferred.

This position requires the manual dexterity sufficient to operate phones, computers, and other office equipment. The position requires the physical ability to kneel, bend and perform light lifting. This person must have the ability to write and speak clearly using the English language to convey information and be able to hear at normal speaking levels both in person and over the telephone. Specific vision abilities required by this job include close vision, depth perception and the ability to adjust focus. Generally, the working conditions are good with little or no exposure to extremes in health, safety hazards and/or hazardous materials. This person must have the ability to travel as required to work with staff.
