YOUTH DIVERSION TEAM CASE MANAGER 4.1 4.1

out of 5 stars Norwich, CT General Description This is responsible youth work involving case management on a Youth Diversion Team program. Work involves assistance in collaborative efforts to implement protocols that guide referral decisions, screening and assessment, and service matching. Work also involves coordination of referrals, case management, and supporting facilitation of the youth diversion team using the restorative justice model. Duties & Responsibilities Regularly attend statewide program meetings. Apply developed policies and procedures to the Youth Diversion Team process. Provide case management services to youth & families. Maintain client files. Support the facilitation of the Youth Diversion Team using the restorative justice model.

SUPERVISION RECEIVED

Works under the direction of the Youth, Family and Recreation Manager, and/or their designee. Qualifications Bachelor’s degree in social services, criminal justice or related field. Experience working with at-risk youth & families, preferably in juvenile diversion setting. Experience with case management. Important Information Confidentiality – the YDT Case Manager may be exposed to confidential medical or behavioral health information. Information learned in the course of employment is strictly confidential!
