Sign-on Bonus:
$2,000 Working in conjunction with the Charlotte County Sheriff’s Office, Employee will be responsible for the outreach, coordination, education and follow up of services for Charlotte County’s Senior Residents determined to be in need of services by a Charlotte County Deputy. Employee will be responsible for identifying the needs and assisting resident in securing appropriate services to ensure the health and safety of clients. Employee will work closely with The Charlotte County Senior Services Department, Department of Children and Families and other local, state and federal service providers to coordinate services for individuals determined to be in need. Employee will make appropriate referrals, monitor the implementation of services, and provide support and advocacy for the individual. Employee will coordinate with caregivers and family members in reference to individuals needs as deemed appropriate. Employee will utilize community resources and regularly contribute updates to these resources so that others might use them as well. Employee will also work with the mental health unit to coordinate services and resources for individuals with mental health and / or substance abuse needs. Bachelor’s Degree from an accredited university or college AND three years full time or equivalent experience working with adults experiencing cognitive related issues. Ability to pass Level II Background Clearance, CCSO background clearance, and drug test. Valid FL drivers’ license, insurance, and safe driving record. Dependable transportation (registered, safe operating conditions, etc.). Able to manage stressful situations and display appropriate work demeanor and boundaries. Demonstrate excellent customer service. Must maintain high standards of ethical and professional conduct, while adhering to agency policies and procedures Must have valid Florida driver’s license that reflects a safe driving record and use of personal vehicle Must have good computer skills in order to enter and retrieve information into an electronic medical record Strong oral and written communication skills. Ability to work both independently and as part of a team
