Jacksonville, FL Job Details Full-time $16 – $19 an hour 18 hours ago Benefits Health insurance Dental insurance 401(k) Paid time off Vision insurance Retirement plan Qualifications Addiction counseling Patient monitoring Social work Mid-level Driver’s License Bachelor’s degree Computer skills 2 years Behavioral health Full Job Description Targeted Case Manager The Case Manager will conduct client screenings, provide assessments, planning, linkage, advocacy and monitoring services to clients who have serious emotional or chronic mental illness, while working closely with their families to maintain them in the least restrictive setting. Essential Functions Assess the needs of each client, develop and implement a goal-oriented service plan to stabilize the client in the community environment. Link to needed services and monitor those services while advocating for needs of the client. All assessment, coordination, and monitoring of services will be conducted in the community and requires driving to the clients homes and to other agencies. Professionally develop a knowledge-based, comprehensive case management system of service enabling the client to function at an optimal level in the home and community. Ensure on-going quality assurance/utilization and compliance with the agency’s policies and procedures and Medicaid standards. Ensure the confidentiality of all client records Involve the client and family in planning to appropriate responsibility. Initiate compliance and follow through on participation in the treatment programs and service plans. Develop an appropriate service plan within 30 days of enrollment Conduct regular monthly visits with each client (at a minimum 32 Billable Hours per week) and provide in-home crisis intervention in the event the client’s is at risk of hospitalization. (PT case managers 30 hours minimum a week) Ensure proper documentation of pertinent information involving the client and family’s progress, including a written case progress report from the involved providers and/or residential facility in accordance with established procedures. Complete accurate documentation, reports, service plans and activity reports in a timely manner as is outlined in the case management general procedures and by their due dates. Assist in formulating and arranging appropriate discharge services/treatment, residential, educational, medical, legal, and social services for client in the caseload. Ensure each case file meets with acceptable standards as set forth in the agency’s policies Case management procedures, quality improvement program, Medicaid, and other required guidelines. Inform the Case Management Director of any significant changes in the case plan, especially regarding projected date of discharge from placement. At a minimum, review of clinical records and monitoring for evaluated progress and readiness for discharge should take place with both the child and professional staff.

Job Qualification Requirements Knowledge/Experience:

Bachelor’s degree from accredited college/university with emphasis in recognized human services field At least two years’ experience in Mental Health or related field Must be certified as State Targeted Case Manager (or within 90 days of hire) Must be able to provide own transportation. Must be eligible for Level 2 Background Screening approval. A valid Florida driver’s license and good driving record. Reliable/ Fuel efficient vehicle and Proof of liability insurance Deals with confidential information and/or issues using discretion and judgment. Must be available and willing to travel to clients (Keystone Heights, Green Cove, Melrose) Good computer skills Full benefits package offered.
