Position Overview:
The Case Manager is the main point of contact for enrolled clients in the Ready4Work program. Clients from a variety of referral sources such as the Department of Corrections, county jail, pre-trial detention, self-referral, and community partners are matched with a Case Manager within their first day of enrollment. The Case Manager completes a Plan of Care with the client to identify short term and long-term goals and monitors the client’s progress toward those goals for one year.

Client Services Manager Essential Duties and Responsibilities:
Manages a caseload of 40+ clients. Completes Plans of Care, including scheduled updates. Completes Risk Assessment with clients. Must be able to communicate professionally and effectively with staff, clients, family members, and employers as needed. Maintains documentation quality to meet requirements including weekly and monthly metrics. Participates in multidisciplinary staffing meetings as part of the client’s team which consists of Mental Health, Career Development and Employment Services . Assists in administering drug tests. Completes termination summaries and assists in discharge planning. Other duties as assigned.

Qualifications:
Bachelor’s degree required (Criminal Justice, Social Work, Sociology, or related fields). Master’s degree preferred. At least 4 years’ experience working as a Case Manager. At least 3 years’ experience working with marginalized or underserved populations. At least 1 year of experience working with clients with mental health and/or substance abuse disorders. Excellent verbal and written communication skills. Proficiency in using computers, running reports, and learning new software. Proficiency in Microsoft Office Suite; Word, Excel, PowerPoint, Outlook, Teams, and electronic client records required. Must be able to work Monday through Friday 8:30am-4:30pm. Must have a valid driver’s license, clean driving record, and reliable transportation. Must be able to clear a Level I Background check through the Department of Corrections. Must be able to lift up to 15 lbs.
