SNAP®, which stands for Stop Now And Plan, is an evidence-based cognitive behavioral model that provides a framework for teaching children struggling with behavior issues effective emotional regulation, self-control and problem-solving skills through structured, curriculum based groups. Youth ages 12-19, experiencing behavioral problems at home, at school, with persons in authority, and in the community can benefit from the SNAP Youth Justice. The program is age specific and works in a group setting to learn skills that teach impulse control, emotional self-regulation, and problem solving skills. The groups meet once per week for thirteen consecutive weeks. All referrals come from the Department of Juvenile Justice Office of Probation. Knowledge:

Knowledge of SNAP Program Policies, Procedures and Performance Standards. Knowledge of human behavior and performance; individual differences in ability, personality, and interests; learning and motivation; and social and psychological norms. Knowledge of group dynamics and how to manage . Knowledge of data collection methods for evaluating program effectiveness.

Skills :

Critical thinking and problem solving skills Highly organized and the ability to multi-task Detail oriented and strong documentation skills. Excellent communication skills (verbal and written) are essential. A solid knowledge of the rules of grammar and punctuation is required. Basic computer skills that allow for creating Word and Excel documents are required. The ability to upload videos to Dropbox and use Video Conferencing Programs like Zoom is required. SNAP Youth Justice Case Manager:

Completes staff training as required by SNAP® and agency Completes intake and discharge assessments for SNAP® for Youth including all required pre and post-assessments (SSIS, SSIS 2, and HIT) Responsible for the day-to-day case management of SNAP® for Youth participants Follows up on incoming referrals and schedules clients for intake appointments Monitors youth (and family’s) progress through weekly check-in calls Facilitates SNAP® for Youth curriculum within the fidelity of the model Liaises with relevant community stakeholders, including schools, community-based agencies, and law enforcement, to enhance outreach Completes all ongoing data reporting requirements and ensures entry to all relevant systems is in a timely manner including NETMIS, JJIS, OneDrive, and SNAP for Youth Data Spreadsheet Ensures weekly materials are prepared and ready for SNAP® for Youth groups, including handouts, manuals, and all other required items Supports the SNAP® Coordinator with the organization of group arrangements as needed Participates in pre-briefing and de-briefing meetings with other SNAP® for Youth Team members Participates in weekly consultation calls with the Florida Network Prepares technology that is required for weekly group delivery (Raspberry Pi, Tablets, projector/TV, camera, etc.) Utilize a strength-based perspective to help youth and families strengthen and manage the quality of their lives by engaging the family, advocating on their behalf, initiating change agent activities, helping them access support in the community, teaching problem-solving skills, modeling productive behaviors, and assisting in the successful completion of youth (and family) developmental milestones Works under the supervision of the SNAP® Coordinator and participates in staff meetings as requested Participate in monthly team meetings, bi-weekly consultations, and staff meetings Assist with client and family transport as needed Attending required training as determined by the SNAP Program contract Responsible for completion of training objectives within the established timeframe All other duties as assigned Education and Experience A high school diploma with a minimum of 3-5 years of human service experience is required. A Bachelor’s degree in Social Work or a related field is preferred. Experience with structured groups and parent education groups is preferred; community/agency liaison experience is desired. Mathematical Skills and Reasoning Ability Basic and/or advanced computational skills may be necessary. Physical Demands This position primarily involves sedentary work, with approximately 70% of the time spent seated. The role may occasionally require lifting or carrying items weighing up to 25 pounds.
