No The Case Manager is responsible for: Case Managers provide direct behavioral case management services to adults or children with serious mental health conditions, substance use disorders, and/or those whose involvement in the child welfare system requires.
The Case Manager should demonstrate competency in the domains of Engagement and Assessment; Service Planning and Delivery; Coordination, Linkage and Monitoring; Documentation; and Professional Responsibility.
The Case Manager coordinates the service delivery for individuals, and provides community partners with up to date available referral resources to provide continuity of care.
Case Managers effectively maintain and manage their assigned caseload, contingent on needs, strengths, abilities, barriers, and preferences of the individual served.
The Case Manager possess a clear understanding of the service delivery system in accordance with regulations and district policy.
Case managers provide services at the time and location that is most appropriate to the person served. Travel is required.
Case Managers may also be required to conduct urine drug screening and transport clients, depending on the program.
Some programs may also require a 24 hour on-call rotation. Case Managers will perform other duties as assigned.
Required Education:
Bachelor’s degree related to related fields, Social Work, Counseling, Psychology Criminal Justice, Health Education, Special Education OR Other Human Services Related Field.
Required Experience:
For targeted case managers working with children have 1 year full-time experience working with children with behavioral health concerns.
For targeted case managers working with adults have 1 year full time experience working with adults with behavioral health concerns.
Preferred Experience:
1 year of full-time experience with the identified population
Required Certification/License:
BLS (Basic Life Support) Driver’s License For targeted case managers (CBHCM) Certified Behavioral Health Case Manager certification must be obtained through the Florida Certification Board (FCB) within one year of employment. Preferred for all case managers.
Targeted Case Managers required to bill Medicaid.
