Full Job Description Position Summary :
The Case Manager is responsible for ensuring the clients discharge plans are appropriate, coordinated and are followed through from admission through all levels of care and thereafter. This position will act as a continuum specific liaison and resource for alumni thereby fostering long-term relationships with the organization. Riverside Recovery of Tampa provides a multi-directional discharge plan designed to continue the patient’s successful journey of recovery and well-being.
Essential Duties and Responsibilities :

Coordinate with clinical team, medical team and patient to identify needs following services at Riverside Recovery Coordinate admission/intake/appoints with follow-up services Development ongoing relations with external providers in various fields Facilitate groups as assigned and deemed competent Documentation in patient records Correspondence with external entities Refer clients to local community health entities (psychiatrist, therapist, Primary Care Physician) Obtain medical records; Marchman Court documents; and

information Assist in facilitation of the discharge process for each client and assist with aftercare planning Update discharge documentation daily to meet all Joint Commission and DCF requirements Attend daily billing/admission/clinical team meetings Update tracking spreadsheet on daily basis Assist clients in obtaining necessary resources to achieve case management goals Maintain client related data systems, including case notes and complete discharge planning entries Adheres to strict boundaries and professional ethics in the care of others Assist in facilitation of the discharge process for each client and assist with aftercare planning Link clients to community resources, and develop relationships throughout the community Ethical and professional standards in compliance with company policies Must be able to develop a network of community resources as it applies the substance abuse and addiction treatment industry
Required Qualifications:
BA/BS Degree in Human Services, Healthcare, or related field One year of experience as discharge planner or related position (i.e., Case Manager) Knowledge and use of EMR Software; Kipu, A general knowledge and background in substance abuse/behavioral health Ability to develop rapport with patient, family, support system and external resources Must have good oral and written communication skills Full time
