Job Description

Case Manager Mary and Martha House Ruskin, FL 33570 Job Summary The Case Manager plays a vital role in supporting individuals and families by providing comprehensive case management services. This position involves assessing client needs, developing personalized case plans, and coordinating resources to facilitate access to essential services. The ideal candidate will possess strong skills in crisis intervention and motivational interviewing, with a focus on improving client outcomes in various areas such as behavioral health, child welfare, and workforce development. Duties Enforces all rules and regulations of Mary & Martha House Programs. Coordinates, cooperates, and communicates with outside agencies in a positive, effective and professional manner. Determines appropriate action/direction for potential clients based on phone intakes. Conducts inner-office intakes and follows preventative procedures with all individuals entering the facility. Provides supportive individualized case management services to residents as needed/deemed appropriate to support their goals toward self-sufficiency, and to assist them in overcoming barriers. Advocate for clients (and their families) by networking and seeking referrals with various social services agencies. Assist clients with budgeting and credit repair. Complete and maintain on a regular basis all necessary client records, documentation, and forms. Complete and maintain on a regular basis all Clarity reports and maintain files for audit purposes. Attend off-site (and conference call) Clarity meetings and training as well as other industry-related training as deemed necessary by Director. Maintain Clarity documents to pass audits. Work with Housing/Apartment managers Provide, maintain and follow-up on all resident assignment sheets, daily work sheets, various records, and chores sheets. Assist with shelter room-ready requirements on an as-needed basis when necessary, such as cleaning out rooms, making beds, etc. Conduct daily room and house inspections of all properties. Flexibility with schedule to transport clients in company vehicle when necessary. Attend and participate in Mary & Martha House fundraising and marketing off-site events. Flexibility required for other duties as assigned or needed Requirements Preferred Bachelor’s degree and/or experience working in the social services field, minimum of three years (non-profit preferred) Must have proficient knowledge in Microsoft Office, Outlook, Excel or similar Must have excellent written and verbal communication skills Ability to work independently in a fast paced environment Knowledge of and sensitivity to cultural, ethnic, racial and socioeconomic issues including working with homeless individuals in a non-judgmental manner with sensitivity to their unique situation. Must have strong organizational skills and attention to detail Must have full availability and be able to work days, evenings, nights, weekends, and holidays This position offers an opportunity for meaningful engagement with clients while making a positive impact on their lives through effective case management practices.

Job Type:
Full-time Pay:

$16.00 – $17.00 per hour Expected hours: 40 per week
