Are you passionate about transforming lives and empowering individuals on their mental health journey? SHSS is seeking dedicated and compassionate Targeted Case Managers to join our team. In this role, you’ll be at the forefront of providing essential mental health case management services, ensuring clients receive the support and resources they need to thrive.
What You’ll Do:
- Conduct comprehensive assessments to understand client needs and develop tailored service plans.
- Coordinate and monitor services, linking clients to appropriate community resources.
- Maintain accurate and timely documentation in compliance with Medicaid standards.
- Engage in regular home visits, client reviews, and collaborative meetings to support client progress.
- Stay current with certifications, training, and professional development opportunities.
Qualifications:
- Bachelor’s degree in psychology, social work, counseling, criminal justice, special education, or a related human services field.
- Minimum of 1 year of experience in a related role.
Florida Certification Board:
Case Management Certification (Preferred).
- Strong verbal and written communication skills.
- Valid driver’s license.
- Bilingual (Preferred)
