Summary The Housing Case Worker for the Transitional Housing Program plays a key role in supporting youth ages 18-24 who are experiencing homelessness. This role requires an energetic and engaging professional who fosters a welcoming, trauma-informed, and supportive environment. The Case Worker collaborates with the housing team and community partners and provides weekly one-on-one case management to assist youth in setting and achieving goals related to education, employment, life skills, health, and self-sufficiency.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Build positive, trust-based relationships with youth clients using a trauma-informed, anti-oppressive approach. Develop individualized case plans aligned with residents’ personal goals in areas such as education, employment, life skills, health, and wellness. Provide consistent, intensive case management through weekly one-on-one sessions and ongoing support. Maintain regular communication with the housing team through thorough case documentation, verbal updates, and weekly staff meetings. Support residents in developing financial literacy, including budgeting and personal finance management. Assist youth in identifying and securing safe, stable, and affordable permanent housing prior to program exit. Participate in comprehensive discharge planning, including aftercare follow-up and continued advocacy as appropriate. Share in a rotating 24/7 on-call schedule with other team members to respond to crises and emergencies, with an expectation to arrive onsite within one hour if necessary. Maintain accurate and timely case notes, intake and discharge summaries, consent forms, and related documentation in accordance with agency and funder standards. Monitor and evaluate resident progress toward goals, adjusting plans as necessary to support continued development. Advocate for client access to necessary resources, services, and benefits to promote independence and well-being. Make appropriate referrals to external agencies and community-based services, including healthcare, education, employment, and life skills development programs. Participate in agency and community meetings, professional development trainings, and team collaborations. Represent the program positively in public forums, outreach activities, and partner agency meetings. Contribute to team productivity goals and uphold program expectations through independent judgment and consistent follow-through. Perform other related duties as assigned by the Assistant Director.
OTHER RESPONSIBILITIES
Complies with all applicable training requirements. Complies with all company safety, personnel and operational policies and procedures. Complies with work schedule to ensure effective operations of Agency programs. Contributes positively as a member of a productive and cooperative team. Participates in Agency Performance Quality Improvement (/PQI) program. Performs other duties as necessary to fulfill Catholic Charities Diocese of St Petersburg, Inc Mission.

EDUCATION AND EXPERIENCE
Bachelor’s degree in social work, psychology, or a related field. Equivalent experience in disaster relief or case management may be considered. Strong leadership and networking skills 1-2 years of case management experience, ideally in disaster relief, emergency services, or social services. Thorough knowledge of community services and target populations. (Optional) Certifications in trauma-informed care, or related areas are a plus.
