Job/Task Functions Responsible for the oversite of case management programs in Lee County, ensuring services and documentation meet standards. Responsible for training including for new case managers, changes to policies and procedures, and in problem areas for all case managers. Provides assistance to individuals who exhibit serious impairment due to mental illness. Work with clients, their families, and significant others to ensure access to the full array of need and appropriate services and resources available in the community to reduce/eliminate the need for institutionalization, hospitalization, incarceration and to enhance independence, autonomy, and quality of life. Provide services which shall include assisting clients in gaining access to needed financial and insurance benefits, employment, medical, social, education, assessment of functional abilities and needs, and other services. These support services include working with the client and his/her natural support system to develop and implement the service plan. It also includes follow-up to determine the status of services and the effectiveness of activities related to the successful implementation of the service plan toward enhancing the recipient’s inclusion in the community. Duties and responsibilities Direct Client Care Provides direct assistance to identified client in the accomplishment of activities of daily living, routine program chores, and personal service plan goals. Demonstrates and coaches the development of skills identified as goals in the service plan. Provides transportation for identified client to maintain medical or other appointments outside the program. Develop weekly schedule of activities for clients based on their needs that ensures consistency. Responds appropriately and in compliance with agency Policies and Procedures to emergent incidents/accidents that develop with client and/or in the environment of care by contacting and reporting to 911 and/or chain of command. Engage identified client in community integration by participating in local activities with client (e.g. Library, attending community outings and events). Communicate with identified collaborative agencies regarding any changes in mental/physical status. Complete all assigned documentation through communicating information to program staff or/and completing a progress note and uploading required documentation into client’s Electronic Medical Record and other human services system if required by position within 24 hours of service. Maintain safety, security, and satisfaction of identified client by monitoring environment of care. Screen, assess, orient, and facilitate the entry of shelter and transition residents into housing units. Develop individualized service plan including identification of existing barriers to self-sufficiency, setting housing and income goals, and developing an action plan. Coordinate and advocate for the involvement of appropriate professionals and resources from other agencies. Meet with clients on a regularly scheduled basis to discuss, assess, direct, and assist in the removal of barriers to self-sufficiency. Assist clients to apply for and maintain benefits. Schedule and conduct house meetings at least monthly. Screen potential new clients in acute care facilities and the community. Meet with professionals from outside agencies to conduct outreach for various programs and collaborate for program success. This may include regular meetings with various groups. Prepare for and participate in audits for case management programs. Maintain census of housing programs in collaboration with property management team. Complete a monthly peer review. Complete monthly housing checklist with each resident. Conduct outreach in the community with the homeless population. Responsible for working with team on clearing out resident belongs after resident vacates property. Supervisory Oversee case managers in Lee County regarding the implementation and success of all the above areas. Supervise case management/supportive housing team with diverse areas of expertise including job development. Provide case consultation with team members on clinical issues and care decisions when resident is presenting with disruptive behavior, looping in the Gulf Coast Regional Manager in a timely manner. Analyze program data and plan for program improvements. With support from the Regional Manager submit monthly and/or quarterly reports with quantitative and qualitative information. Provide supervisory support for Gulf Coast Region team members when Regional Manager is out of office, to include other team lead(s). Review/Audit of client electronic files and data systems to ensure case management plans address identified needs, are up-to-date and that data quality is maintained. Ensure these records meet standards and are in compliance with regulatory agencies. Ensure case managers continually monitor client activity related to achieving personal and programmatic goals and developing corrective action revisions to case plans as needed. Conduct or arrange for appropriate orientations, in-services, and training for program staff, including training new staff, to ensure compliance with standards and regulatory agencies. Communicate regularly with supervisor regarding any ongoing staff issues, including performance and behavioral problems. Complete warnings and disciplinary action when needed according to CASL policies and standards. Conduct regular performance evaluations of supervised team members according to CASL standards in conjunction with Gulf Coast Regional Manager. Maintain related administrative record keeping and reporting systems as required by contracts. Maintain accurate documentation of service objectives and outcomes as well as other services in accordance with Federal, State, County, and CASL guidelines. Prepare case management related reports including but not limited to outcomes, goals, successes, etc. Adheres to strict boundaries and professional ethics the care of others. Provide training as needed for any policy or procedure change. Provide ongoing training and guidance to team members in any problem area identified. Ensure coverage of case load, including documentation, for case managers during any sick, vacation, other leave or when short staffed. Interact professionally with residents, co-workers, and community agencies. Must be able to maintain tact and professionalism. Check company email while on duty, responding timely, appropriately, and professionally to communicate internally and externally. Other duties and responsibilities as assigned.
