Purpose and Impact:

Provide trauma-informed case management services and referral provision to assist clients in reaching their goals.

Essential Functions:

Provide comprehensive case management services to program participants by researching community partners and making referrals to appropriate social service agencies, community-based organizations and/or social service providers. Recruit program participants through internal referrals and outreach to eligible individuals. Complete eligibility screenings for all participants recruited by or referred to the PC Program. Conduct comprehensive intakes for all participants determined eligible for the program. Develop goal-oriented, client-centered self-sufficiency plans with each enrolled program participant, addressing their long- and short-term needs and goals. Provide extended/enhanced cultural orientation workshops to program participants as needed. Guide program participants through referrals, providing follow up, check-ins and assistance, ensuring all services are linguistically and culturally appropriate. Screen for direct participant financial assistance eligibility in accordance with LIRS basic needs direct assistance practice, facilitate disbursement and conduct all necessary follow-up and associated financial reporting. Maintain regular contact and positive rapport with all PC participants. Collaborate closely with other LSF case managers to facilitate PC enrollment of referred participants and coordinate services to dual-enrolled participants. Provide ongoing guidance to and communication with volunteers matched with PC clients for English tutoring, career mentorship and more, as needed. Participate in PC community coordination, capacity building and partnership development activities in the service area. Complete case notes and required assessments in online database in a timely and consistent manner. Maintain comprehensive case files for each enrolled participant, ensuring client confidentiality and protection of personally identifiable information. Acquire comprehensive knowledge and thorough understanding of programmatic requirements of PC Program. Attend ORR, LIRS, LSF, and other resettlement agency workshops and trainings centered on trauma-informed case management, PC specific vulnerability support, community-based mental health support, economic self-sufficiency and more. Trains, supervises and supports PC Program Interns and volunteers. Prepares PC Program reports as required. Performs other related duties as assigned.

Other Functions:

May track all financial transactions of the client’s funds as per accounting procedures in a timely manner May create and document check request vouchers for all clients per scheduled if necessary Must possess a clean/valid Florida Driver License, reliable transportation and proof of insurance (transcript of driving record will be requested from the Department of Motor Vehicle at the time of hire). Assists manager with RFP and other project. Assists Manager in attaining program goals. Provides training to program staff if necessary.

Physical Requirements:

Ability to remain in a stationary position (sitting or standing) for extended periods of time. Ability to operate a computer and other standard office equipment (telephone, printer, copier, scanner). Ability to communicate clearly and effectively in person, over the phone, and in writing. Frequent movement inside the office and to off-site locations, including client homes, community centers, and partner agencies. Ability to lift and transport items such as client files, donations, or supplies, up to 25 pounds. Ability to travel independently within the service area using personal transportation, including walking short distances and climbing stairs as needed. Visual and auditory ability to observe and respond to client needs, read printed materials, and interact with digital platforms. Occasional exposure to varied environmental conditions during outreach, home visits, or community events. Education:

Bachelor’s degree in a Social Work or behavioral/human services or related field required.

Experience:

Minimum 1-2 years case management or direct service experience required, preferably working with refugees, immigrants or vulnerable populations. Cross-cultural or international experience preferred. Knowledge, Skills & Abilities Proficiency in Microsoft Office applications and case documentation tools. Knowledge of community resources and ability to connect families to community services. Excellent verbal and written communication skills Critical assessment and analysis skills Ability to work and deal a diverse group of clients. Must be able to keep confidential information. Availability to work occasional evenings or weekends based on client needs. Bilingual skills are required.

Principal Accountabilities:

Reports to Manager Must be a team player with other staff as well as internal and external partners. Accurate, complete and timely submission of required reports and documentation to

LIRS & LSF.
