Case Management Associate Reports to:
We Care Network Program Coordinator Employment type: Full-time (Non-exempt) We Care Network Case Management and Dental Case Management Associate The We Care Network is a charitable program of the Capital Medical Society Foundation, a non-profit organization operating alongside the Capital Medical Society. Its mission is to provide a safety net to those low-income, uninsured patients most in need in our community through the donation of specialty medical care and dental care. (Qualifications for patient acceptance: Uninsured adults whose household income and expenses must be at or below 150% of the Federal Poverty Guidelines. Patients cannot qualify for any alternative programs (Medicaid, Medicare, etc.) that will pay for their care.) Over 300 doctors and more than 40 dentists, the local hospitals, and numerous ancillary partners provide well over $7 million of free care annually to very low-income, uninsured patients. We Care Network case managers screen, qualify, and obtain appointments for patients referred by primary referral sites. Limited funding is used to assist with transportation and prescriptions for these patients.
General Description:
The We Care Network Case Management Associate assists Case Managers and patients referred to the CMS Foundation’s We Care Network. The individual in this position creates an organized flow of referrals, questions, phone calls, and documents from the primary care providers, patients, and donating providers to the Case Managers. Responsibilities include (but are not limited to) assisting case managers with the referral screening process, entering data into the database, greeting walk-in guests, answering and directing phone calls, administering patient satisfaction surveys, and maintaining positive communications with physicians and dentists who donate care. We Care Case Management Associate Essential Duties and Responsibilities include the following.

Greet walk-in guests.
Faxes.
Assistance to Case Managers.
Open mail.
Value of Services.
Attends meetings of the We Care Network Advisory Committee and We Care Network Dental Committee
Attends other Capital Medical Society Foundation functions as requested
Financial eligibility screening and coordinating care for patients referred. This includes answering phone inquiries, determining eligibility, scheduling appointments.
Preparation for and attendance of Dental Clinic Days as needed.
Maintain and update the database, including the value of invoices from dental providers.
Participate in the compilation of statistical data needed for assigned reports and invoices required by funding agencies with the support of the Dental Case Manager and Program Coordinator. •We prefer to promote from within, so if a Case Manager position becomes available in the future, this position is offered that opportunity prior to posting an external advertisement•
Requirements:
A Bachelor’s Degree. Social Work preferred.
Commensurate experience for position requirements.
Ability to drive to work-related locations with operational, personal transportation.
A Valid Driver’s License.
Proof of Current Personal Auto Insurance Coverage at minimum limits of $25,000/$50,000/$25,000. •Requirement may increase as required by the organization’s liability carrier.
Basic medical terminology.
Experience with the local medical community highly desirable.
Bi-lingual in Spanish and English preferred but not required.
