POSITION OBJECTIVE
To ensure young adults who are involved in the dependency system have the necessary knowledge, skills and resources to transition to independence by providing case management services and advocating for their best interest. Independence should be promoted by supporting the young adult’s educational success, development of employment skills, establishing a network of supportive adults, along with an overall focus on well-being of the young adult.
ESSENTIAL FUNCTIONS
- Serve as primary case manager for young adults age 18 -23 who are active Extended Foster Care, Postsecondary Education Services and Support (PESS) or Aftercare Services.
- Prepare and process all paperwork to establish and monitor eligibility for Extended Foster Care, PESS and Aftercare Services.
- Document all case activity in the Florida Safe Family Network (FSFN) and ensure case file contains appropriate eligibility documentation.
- Engage young adults in developing life plans and setting goals in areas of education, housing, transportation, personal finance, health and wellness; assist in accessing resources needed to meet plans and goals.
- Establish and maintain communication with community care agencies, case management agencies and dependency programs.
- Establish and maintain strong working relationships with educational institutions and community businesses to facilitate client development.
- Develop and maintain clinical records and program documentation according to the contract and NWFHN standards (assessments, treatment plans, progress notes, termination summaries, etc.).
- Prepare Case Plans and Judicial Review Social Study Reports and attend court for young adults in Extended Foster Care.
- Attend court as secondary for youth age 16+ that will likely enter IL program.
- Meet monthly with and maintain an open line of communication with clients.
- Prepare and assist in the delivery of independent living lessons including but not limited to employability training, vocational skills, budgeting/financial skills, nutrition/meal preparation, and socialization/interaction skills; encourage client participation in community presentations.
- Facilitate assessments of each youth/young adult’s current level of life skill achievement and assist in developing plan to increase areas of deficit.
- Be responsive to include 24 hour crisis intervention to young adults in Extended Foster Care, PESS, and Aftercare Services.
- Manage initial and subsequent placements for young adults in the Extended Foster Care program to include all required assessments of placement, living agreements, and maintain FSFN record for eligibility purposes.
- Support case managers in developing transition plan for the youth who are 16 and 17 years of age.
- Keep abreast of all dependency, foster care, and independent living statutes and contract requirements.
- Participate in client case planning meetings.
- Represent the program at community meetings.
- Arrange for or transport clients as necessary; maintain NWFHN vehicle.
- Assist with program coverage and/or administration as directed.
- Ensure credit checks are properly provided to youth and young adults and assist the young adults in understanding how to read report as well as the importance of credit.
- Participate in the Quality Improvement process.

QUALIFICATIONS
:
- Bachelor’s Degree in a Human Services related field from an accredited university or college (All degrees must be from an accredited university or college) or a combination of education and experience required for eligibility for a child welfare certification by the Florida Certification Board. - Possess State of Florida Child Protection Certification
- Two years’ experience working with young adults
- Valid driver’s license
- Knowledge of local community resources
- Experience working within the child welfare system

Qualifications:
A high school diploma or GED equivalent and four years of law enforcement experience or active military service (honorably discharged or status that current service reflects serving honorably); or An associate degree or 60+ credit hours from an accredited college or university and two years of professional work experience (see examples below) or two years of law enforcement experience or active military service (honorably discharged or status that current service reflects serving honorably); or Examples of professional work experience could be, although not limited to : Guardian Ad Litem or similar child advocate role Family Support Worker Group Home Worker Teacher’s Assistant/Aide Daycare Provider/Worker Therapeutic Assistant Behavioral Health Technician Family Intervention Specialist Home Health Aide Nurse (LPN or RN) or similar profession Nursing Facility Assistant EMT Other welfare, education or medical professional jobs that occur in high-paced, high-stress environments that require critical decision-making to occur A bachelor’s degree from an accredited college or university. An official letter from the college/university registrar or unofficial transcriptions with anticipated graduation dates and credits earned must be provided for all diploma/degree requirements. Proof of conferred degree such as official transcripts or copy of diploma/degree must be provided within 30 days of hire as a condition of employment. Successful candidates must obtain a Florida Child Protective Investigator Certification from the Florida Certification Board within 12 months after obtaining provisional certification.
