We are excited about recruiting for the position of Case Manager who facilitates internal client assistance programs as well as conducts triage of incoming community-based client and caregiver needs. Our ideal candidate will be an experienced social work professional with an in-depth knowledge and practice of community advocacy, excellent communication skills, and the ability to organize ever changing priorities while effectively meeting the needs of the clients. Responsibilities 1. Process referrals and complete initial client assessments in order to identify: a. Problems and immediate needs of the client b. Client’s strengths and ability to cope with his/her limitations c. Caregivers and/or agencies providing assistance d. Gaps requiring intervention 2. Assist client enrollment in appropriate program including: a. Educating client about programs available and qualifications b. Assist client in completing appropriate application/paperwork for enrollment c. Assure client enrollment requirements are met 3. Provide information and counseling as needed to client and caregiver. 4. Involve the client in the care plan development. 5. Develop care plan and assure care plan approval is obtained. 6. Arrange, coordinate, and monitor client services outlined in care plans either through ECS or outsourced provider. 7. Review care plans with other professionals involved with service provision to the client. (Outsource providers, senior companions, etc.). 8. Provide follow-up (as per program requirement) to assure client satisfaction. 9. Complete client visit and contact as per program requirements and time frames. 10. Review the service delivery impact on the client as per program requirement but at a minimum of every six months. 11. Complete a written client reassessment annually (within 365 days). 12. Complete annual paperwork in timely manner (within 365 days). 13. Complete case notes and assure billing of case management services in timely manner. 14. Maintain caseload/unit production, program standards and goals. 15. Assist clients in obtaining equipment and supplies (including ordering supplies in a timely manner). 16. Assist fiscal in requesting client payment of co-pays or advocating for co-pay adjustments as needed. 17. Advocate for the needs of clients and caregivers. 18. Maintain confidentiality and security of Protected Health Information (PHI) according to HIPAA guidelines. 19. Receive and respond to referrals from CARES and Adult Protective Services in required time constraints. 20. Perform other duties as assigned by the Clinical Director and/or the President/CEO. Skills A bachelor’s degree in social work, sociology, psychology, nursing, gerontology or a related social services field is required; or four years of related job experience or any combination of education and related experience may be substituted for a bachelor’s degree upon approval of the Area Agency on Aging. Basic computer skills and knowledge is required. A reliable personal vehicle available and a good driving record, travel is required within in Leon County. Successful completion of Area Agency on Aging for North Florida Case Manager certification as made available by the Area Agency on Aging Employee must successful complete the 701B Assessment training within 10 business days of hire. Available to work hours beyond the usual workday of 8:00 a.m. to 5:00 p.m. when necessary While performing the duties of this job, the employee will spend a majority of time in client home and will be frequently exposed to a variety of patient conditions and elements. The noise level is moderate to quiet.
