This is a full-time, in-office position offering a competitive salary of up to $60,000 and the opportunity to work on impactful cases supporting injured individuals and their families. Job Summary The Case Manager plays a critical role in providing support and guidance to individuals and families navigating complex systems of care. This position involves assessing client needs, developing personalized service plans, and coordinating resources to ensure clients receive comprehensive support. The ideal candidate will have a strong background in social work, crisis management, and workforce development, with a focus on empowering clients to achieve their goals. Responsibilities Conduct thorough assessments of clients’ needs and circumstances to develop tailored service plans. Coordinate services and resources for clients, including referrals to community organizations and support services. Monitor client progress and adjust service plans as necessary to ensure effective outcomes. Provide crisis intervention and management support to clients facing immediate challenges. Facilitate communication between clients, families, and service providers to ensure continuity of care. Mentor and supervise junior staff or interns in case management practices and procedures. Maintain accurate documentation of client interactions, service plans, and progress reports in compliance with organizational policies. Stay informed about relevant programs such as Section 8 housing assistance and Medicare benefits to provide informed guidance to clients. Skills Strong understanding of social work principles and practices. Proficient in crisis intervention techniques and crisis management strategies. Experience with child welfare systems and workforce development initiatives is highly desirable. Excellent mentoring skills with the ability to supervise staff effectively. Familiarity with motivational interviewing techniques to empower clients in their decision-making processes. Exceptional communication skills, both verbal and written, for effective client interaction and documentation. Ability to work collaboratively within a multidisciplinary team environment while advocating for client needs.
