CASE MANAGER
(ADULTS/CHILDREN) To provide ongoing support and expertise through comprehensive assessment, planning, implementation and overall evaluation of individual client needs. The overall goal of the position is to enhance the quality of patient management and satisfaction, to promote continuity of care and cost effectiveness through the integrating and functions of case management, utilization review and discharge planning. Is accountable for the care, coordination and discharge planning of all clients. Must have a minimum of a baccalaureate degree from an accredited university in psychology, Social Work, Health Education or a related human services field; and at least one (1) year experience or more years of full time or equivalent experience working with target population who have serious mental illness/emotional disturbances. Skilled in community organization and familiarity with community resources. Has completed AHCA approved case Management training or will complete it within six (6) months of employment.
