This position involves coordination of patient care and Infusion therapy, assessing patients’ health needs, developing personalized care plans, and facilitating communication between patients, families, and healthcare providers. By monitoring patients’ progress and adjusting care plans as necessary, the Home Health Infusion Case Manager ensures that each patient receives proper treatment as ordered, and the support needed to achieve their health goals. This role not only focuses on optimizing patient outcomes but also emphasizes the importance of patient autonomy and comfort, making it a unique blend of clinical expertise and compassionate care coordination. Requirements Graduate of an accredited nursing program with a current license (in good standing (as a Registered Nurse) of practice. Infusion Experience preferred. One (1) Year home care experience or as required by state law. Knowledge and ability and the necessary skill and judgment to perform all nursing procedures as outlined in the scope of practice in the Nurse Practice Act. Excellent verbal and written communication skills, and strong interpersonal skills. Knowledge of the regulatory requirements at the state, federal, and local level. Maintains productivity standards as established by the corporate office. Absence of criminal background. Job Duties and Responsibilities 1. Manage and coordinate the care for patients receiving Infusion therapy treatment in the Home Setting. 2. Coordinate with physicians, nurses, and other healthcare professionals to implement care plans and ensure continuity of care. 3. Educate patients and their families on managing illnesses or injuries and guide them on using home health care equipment. 4. Monitor patients’ progress and adjust care plans as necessary to meet their changing health needs. 5. Facilitate access to community resources and services, such as support groups or home health aides, to assist patients and families. 6. Document patients’ care, including assessments, care plans, and progress, ensuring compliance with healthcare regulations and privacy laws. 7. Coordinate the management and safe administration of Infusion Medications in the Home Setting. 8. Overseeing the field nurses with the proper education of patients on proper medication usage and potential side effects. 9. Obtain report on home safety evaluations to identify potential hazards and recommend modifications to prevent injuries.
