Job Description
Long Term Care Case Manager First Quality Home Care, Inc. 2001 NW 107th Avenue, Doral, FL 33172 Case Manager has the knowledge and responsibility to assist the consumer in meeting their needs by linking them with an array of community based services and resources. Service arrangement shall not be limited to those services offered by the agency for which the Case Manager work. Case Manager provides linkage between health care and social services delivery systems. Case Manager monitor services to ensure they are having a positive impact on the problems that necessitate the services. Training and certification on Department of Elder Affairs (DOEA) assessment and Care Plan forms is required for Case Manager that we will provide.
Qualifications:
Bachelor’s degree in Social Work, Psychology, Nursing, Gerontology or related field. Job related experience or any combination of education and experience may be substitute for education. Driving required and 1 year of experience working with senior citizen is required. Bilingual, (English and Spanish). Salary based on education and experience.
