Job Description

Outpatient Case Manager 2.5 2.5 out of 5 stars 22790 SW 112th Avenue, Miami, FL 33170 Case Manager – Outpatient (Fee for service) $26.00 Are you looking for growth, great benefits and a flexible work schedule? Do you have a passion for helping others? We are looking for energetic and empathetic candidates to join our Outpatient Case Manager Department. Case Manager duties include assessing, planning, implementing, monitoring and evaluating actions required to meet the client’s health and human services needed. A certified Case Manager is a plus.

Essential Duties and Responsibilities:

Maintain caseload of active clients that require case management in such areas as programming court appearances, doctor appointments, family therapy sessions, and discharge referrals. Act as liaison between courts, families, and outside agencies. Provide case management services in accordance with the service plan as appropriate to the program. Meet and coordinate with the members of the Treatment Team and provide them the appropriate reports prior to meeting. Completes documentation and reports as necessary. Provides on-call services as needed. Attends staff meetings as necessary and participates as a team member. Upholds total confidentiality with respect to clients in all instances. Attends training and in-services as required. Performs other duties as appropriately and assigned. Ensure that each client is registered for food stamps. Follow up on the completion of the process, working jointly with economic sufficiency liaison. Monitor the billing data/units for Medicaid. Maintain client case management logbook in accordance with Program/Contract guidelines

Education and/or experience:

Bachelor’s degree in Psychology, social work, mental health, or related field. One year experience in counseling or case management with Severely Emotionally Disturbed adult/children. Must be able to provide their own vehicle, the vehicle insurance and have current FL driver’s license in case of emergency. Possess targeted case manager certification or obtain certification within the first year of employment.
