PRIMARY RESPONSIBLITY
The primary responsibility of this position is to support the child welfare system of care partners in ensuring timely permanency for families with children placed in out-of-home care. Completing tasks to increase timely permanency (including facilitating chapter 39 Multidisciplinary Team staffing (MDTs)) and decrease re-entry in to out of home care.
EXAMPLE OF ACTUAL FUNCTIONS

Complete family finding tasks, including sibling relationship supports. Complete MDT facilitation tasks as necessary, including coordinating and facilitating multidisciplinary staffing’s in person or remotely, sending notifications to all participants who are required to attend, and completing corresponding documentation and timely follow-ups. Collaborates with FCMAs and internal departments such as Behavioral Health, Nurse Case Management, and Quality Assurance (QA) to formulate action plans to support/ create /stabilize non-licensed placements for children. Host staffing and meetings as necessary to assist in the achievement of timely permanency Assist Full Case Management Agencies with completion of permanency enhancing tasks, with a focus on timely completion and appropriately utilizing the child welfare practice model. Link Full Case Management Agencies with the appropriate Citrus Family Care Network (Citrus FCN) departments to assist in permanency enhancing tasks. Conduct a thorough review of the relevant children/ youth case records with the intent of identifying actions plans to enhance permanency options for children in out-of-home care. Contact with families including youth to explore viable action plans to achieve permanency. Contact with stakeholders and community partners including Full Case Management Agency partners to explore viable action plans to achieve permanency, per family and systemwide. Follow-up on permanency and transition plans as necessary. Document within CWIS all family specific permanency efforts, including contacts attempted and completed. Support and encourage the Quality Parenting Initiative (QPI) principles. Ensure all available community resources have been exhausted first before submitting funding requests. Attend dependency court hearings and administrative hearings when necessary. Must develop expertise and working knowledge in the governing law, rule and polices of permanency, conditions for return, out-of-home placement and in home safety planning. Aware of the various permanency options and assigned benefits for each option including Extension Monthly Maintenance Adoption Subsidy program and the Guardianship Assistance Program. Compliance with established permanency procedures, in collaboration with the Department of Children and Families (DCF) and pursuant to state and federal guidelines Participates in regular supervision, department and multidisciplinary meetings to ensure efficient and effective teamwork, collaboration and solution-oriented approach to addressing barriers and challenges to accessing or stabilizing lifelong family connections for children. Develops and maintain a professional, collaborative relationship with children/youth, caregivers, families and community stakeholders. Participates in the Quality Assurance/Quality Improvement Program. Displays understanding of and sensitivity to service population’s cultural and socioeconomic characteristics. Travel throughout Miami-Dade and Monroe County including co-location at FCMA sites. Ensures timely and quality submission of all relevant work product. Provision of services during flexible hours.
MINIMUM REQUIREMENTS

Bachelor’s Degree from an accredited college or university in Social Work or a related field, combination of child welfare experience and other unrelated degree may be considered. Master’s degree preferred. Must be certified Child Welfare Case Manager (CWCM) within one year of hire. Must complete MDT facilitator training requirements as per Florida Administrative Code within 1 year of hire Familiarity with Florida Child Welfare Practice Model and Child Welfare related computer programs. Knowledge of child welfare principles and practices to include but not limited to trauma informed care. Strong knowledge of HIPAA guidelines, and superior understanding of the importance of confidentiality. Highly developed Cultural Awareness and Sensitivity is required. Must have comprehensive knowledge of a variety of computer software applications in word processing, spreadsheets, database, and presentation software, including Word, Excel, and other MS Office products.
