Afterschool Mentorship Program Summer Mentorship Program Parenting Mentorship Program Men & Women’s Self-Care Program 12-Week Saturday Mentorship Program Job Overview We’re seeking an experienced and compassionate Youth Case Manager to develop and implement our case management program, providing holistic support to at-risk youth. As a key team member, you’ll have the opportunity to build and shape our case management department from the ground up. We’re looking for a strong Christian who embodies our faith-based values and principles, and is passionate about making a positive impact in the lives of young people. This part-time position (30 hours/week, M-F, 9:00am-3:00pm) has the potential to become full-time with benefits, offering a unique chance to grow with our organization. Below, you’ll find key responsibilities for this exciting role.
Responsibilities Key Responsibilities:
Develop and Implement a comprehensive case management program for our youth mentorship program.
Case Management:
Conduct intake assessments, create individualized service plans, and provide ongoing case management support to youth participants. Provide individualized support to youth participants, including goal setting, progress monitoring, and connecting them with community resources.
Mentor Match Support:
Provide guidance and support to mentoring matches, ensuring a positive and productive experience for both mentors and mentees. Monitor progress, identify areas for improvement, and adjust case management strategies as needed. Collaborate with mentors, program staff, and community partners to ensure holistic support for our youth.
Family Engagement:
Develop and maintain relationships with parents/guardians, providing them with updates on their child’s progress and involving them in the mentoring process.
Program Evaluation:
Participate in program evaluation and outcome measurement, using data to inform program improvements. Offering spiritual guidance and support.
Community Outreach:
Establish and maintain relationships with community organizations, schools, and other stakeholders to promote the program and recruit participants. Connect youth with community resources, services, and programs.
Crisis Intervention:
Provide crisis intervention and support to youth participants and their families as needed.
Record Keeping:
Maintain accurate and up-to-date records of participant progress, mentor matches, and program activities.
Youth Assessments:
Conduct assessments to identify youth needs, strengths, and challenges.
Individualized Service Plans:
Develop and implement individualized service plans for each youth participant.
Progress Monitoring:
Monitor youth progress, provide feedback, and make adjustments to service plans as needed.
Training:
Provide training and guidance to program staff on case management best practices. Experience A Bachelor’s degree in Social Work or a related field is preferred; relevant experience may be considered in lieu of formal education.
Experience:
At least 5 years in case management, preferably in a youth-serving organization.
Skills:
Excellent communication, interpersonal, and problem-solving skills.
Certifications:
CPR/First Aid certification and any other relevant certifications. Strong knowledge of case management principles, practices, and software. Excellent communication, interpersonal, and problem-solving skills. Ability to work independently and as part of a team. Familiarity with faith-based organizations and values. Strong interpersonal skills with the ability to build rapport with diverse populations. Experience supervising staff or volunteers is beneficial but not required. Knowledge of childcare practices and resources can enhance service delivery effectiveness.
