Full Job Description SUMMARY:
Responsible for ensuring continuity of care through advocating, planning, linking, coordinating, and monitoring of services and resources to help improve the quality of life for individuals served.

Successful completion of Background, MVR and Drug Free Workplace Screenings. EDUCATION and/or
EXPERIENCE
Bachelor’s degree from an accredited four-year college or university with emphasis in the areas of psychology, social work, health education or related human services field. Preferred one year of full time or equivalent experience working with adults who have serious mental illness and children who have serious emotional disturbances. – OR- Have a minimum of a bachelor’s degree from an accredited university and at least three (3) years full time experience working with children who have serious emotional disturbances or two (2) years full time experience in working with adults who have serious mental illness.
CERTIFICATES, LICENSES, REGISTRATIONS
Certification or certification-eligible by Medicaid as a Case Manager.
SPECIAL SKILLS
Experience in providing case management and advocacy services to individuals in an outpatient mental health setting. Excellent written and verbal skills necessary. Computer literate; trained in electronic health record system upon hire.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Electronically documents case management activities according to Agency, State, Medicaid and other accreditation standards and policy and procedures. Provides program linkages with other agency programs and community resources to coordinate service delivery, remove impediments to effective care, and achieve established objectives. Advocates for and links the individual with identified needed services and resources. Coordinates all services ensuring individual’s needs are being met toward completion of service plan goals. Works with the individual’s family and natural support system in the individual’s natural environment. Monitors and ensures compliance in service delivery and documentation. Participates as a team member and contributes to the enhancement of the department and agency In addition to the duties listed in the Case Management position description, the following are additional duties for the position identified below. These are not to be construed as exclusive or all-inclusive. Other duties may be required and/or assigned as needed. MAR Case Manager Specialty/Population Specific Duties Have knowledge of the Medication Assisted Recovery program, requirements, and phases. Review and monitor Medication Assisted Recovery participant’s appointments with their medical providers, appointments with their therapists, weekly groups, and drug screenings. Coordinate with the Medication Assisted Recovery team including the medical providers, therapist, MA’s, and peers. Co-facilitate the weekly MAR group. Complete inductions for individuals starting the MAT program. Complete GPRA’s for new MAR individuals and input them into the system. Attend the weekly MAR meeting. Complete and send re-commitment and discharge letters for identified individuals. Complete Agency Discharges for identified individuals.
