We prioritize collaboration, compassion, and continuous improvement What You’ll Do As a SOAR Case Manager, you’ll play a vital role in helping individuals apply for and secure Social Security disability benefits through the SOAR (SSI/SSDI Outreach, Access, and Recovery) model. You’ll also connect clients with additional support and resources to promote stability and well-being.

Key Responsibilities:

Conduct interviews and gather information for

SSI/SSDI

applications Write clear, accurate medical summary reports Coordinate with doctors, psychiatrists, and specialists Work closely with referral sources and community partners Provide case management and follow-up services Advocate for clients and ensure continuity of care What We’re Looking For Bachelor’s degree in Psychology, Social Work, or related field Experience working with adults with serious mental illness and/or emotionally disturbed children Medicaid Case Manager certified or eligible Strong written and verbal communication skills Organized, compassionate, and community-minded Additional Requirements Must pass background, MVR, and drug-free workplace screenings Computer literate; training on our electronic health record system provided About the SOAR Program The SOAR initiative helps individuals experiencing or at risk of homelessness obtain critical income and healthcare through Social Security benefits.
