$36.2K – $40.9K a year 1 hour ago Qualifications Microsoft Excel Writing skills Mid-level High school diploma or GED Driver’s License Bachelor’s degree Computer skills 1 year Associate’s degree Full Job Description SUMMARY The HOPWA Case Manager provides case management services to individuals and families living with HIV/AIDS, assists participants in securing permanent housing, and provides referrals for supportive services to community partners, all aiming to maximize self-sufficiency and housing stability. In addition, the HOPWA Case Manager supports building life skills and attaining short- and long-term housing goals.
ESSENTIAL DUTIES AND RESPONSIBILITIES
include the following. Provides housing case management services for assigned clients, including assessment, planning, linking, monitoring, advocacy, and procedures to achieve successful outcomes. Provides housing case management to clients with housing insecurities or eligible housing emergencies. Conducts comprehensive client assessments to collect functional, environmental, psychosocial, financial, employment, housing, educational, and health information as appropriate and develop appropriate case plans. Develop support systems to meet client needs by identifying and coordinating various available services necessary to maintain independent living selfsufficiency and family stabilization when possible. Conducts housing crisis intervention with the unhoused or clients with housing emergencies as HUD guidelines require. Monitors and verifies services provided to each client monthly, determining the quality and effectiveness of services offered. Assess, orient, and facilitate the entry of clients to the HOPWA (Housing Opportunity for People With AIDS) program. Sustains and manages a caseload consistent with the program’s annual contractual obligation by applicable quality criteria. Completes preliminary assessment process for persons seeking services from the agency to collect demographic information and pertinent medical history. Initiates review of prospective client eligibility and continues review for all active clients. Completes and makes referrals for assessments and evaluation processes based on client needs and treatment objectives. Respond to information requests from other specified partner agencies. Visits agencies, organizations, and individuals to promote housing programs as required. Monitors client participation in planned services. Initiates revisions to the plan of care and closes or transfers cases. Obtains authorization for payments from the supervisor and submits all requests for processing according to established time frames. Attends all regularly scheduled staff and case management meetings. Participates in agencywide events that may involve weekends. Performs other workrelated duties and special projects as assigned.

KNOWLEDGE AND SKILL OF
Florida Department of Health 500 and 501 HIV/AIDS certification. Modern office practices, procedures, and equipment. HIPAA laws and confidentiality requirements. Excel, Word, and basic computer operations. HIV transmission, prevention, and disease process.

EXPERIENCE
