care 1 year Care plans Full Job Description SUMMARY Responsible for providing HIV Intensive Case Management services to an assigned caseload in accordance with Federal legislation, the Ryan White Treatment Modernization Act of 2009, and the Orlando EMA HIV Health Services Planning Council. These services help consumers gain access to needed medical, social, educational, and other services. This includes direct assistance in gaining access to services, coordination of care, and linkage to appropriate services.
Medical Case Management includes:
(1) Initial assessment of service needs; (2) Development of individualized care plan; (3) continuous client monitoring and follow-up; and (5) Treatment adherence.
ESSENTIAL DUTIES AND RESPONSIBILITIES
include the following. 1. Initial assessment of service needs 2. Development of a comprehensive, individualized care plan 3. Timely and coordinated access to medically appropriate levels of health and support services and continuity of care 4. Continuous client monitoring to assess the efficacy of the care plan 5. Re-evaluation of the care plan at least every 6 months with adaptations as necessary 6. Ongoing assessment of the client’s and other key family members’ needs and personal support systems 7. Treatment adherence counseling to ensure readiness for and adherence to complex HIV treatments 8. Client-specific advocacy and/or review of utilization of services 9. Make appropriate referrals to Hope & Help’s services and other service agencies. 10. Perform oversight of the entire case and linkage to appropriate services. 11. Maintain accurate, same day records of each client interaction via progress notes and corresponding documentation in the client file and meet all other reporting deadlines specific to grantee guidelines. 12. Coordinate with Referral for Healthcare Specialists for all clients on shared caseloads.
OTHER RESPONSIBILITIES
Performs other work-related duties and special projects as assigned.

EXPERIENCE
Bachelor’s degree in a social science from an accredited college or university with at least one (1) year of case management experience, or Bachelor’s degree not in a social science from an accredited college or university and must have at least one (1) year in direct
HIV/AIDS
case management experience. CERTIFICATES, LICENSES, and/or
REGISTRATIONS
Valid Florida Driver’s License required.
