care Care plans Case service coordination Communication skills Entry level Full Job Description This position is a full time (1 FTE) direct service Medical Case Management position responsible for the provision of advice and assistance in obtaining medical, social, community, legal, financial and other needed services in accordance with the Orlando EMA Standards of Care documents and is directly supervised by the CM Supervisor.
Duties include:
Maintain a minimum of 50 Ryan White eligible consumers for case management services to include Assessment of consumer needs and development of a Care Plan to meet the needs of each client. Maintaining orderly files and document all activities relating to each consumer contact and provide accurate information. Provide referrals for services as appropriate. Prepare and participate in Fair Hearings as necessary (when a client requests to appeal the determination of eligibility) Have knowledge of Federal, State and local programs in order to refer clients. Participating in group and/or multidisciplinary team case conferences. Responsible for documenting case conferences in the consumer’s file in accordance with Orlando EMA Standards of Care. Conducting outreach and engagement activities to potential consumers and service providers to promote the availability of eligibility and linkages to services that support the consumer’s access to and maintenance of primary medical care. MCMs will function along with Peer Mentors as appropriate as consumer navigators to determine barriers to care and connect or reconnect consumers to

services. Enter each client into the PROVIDE Electronic System and update the database as needed. Participating in Medical Case Management Training in accordance with the Orlando EMA Standards of Care. Participating in mandatory and self-selected training and staff development activities in accordance with the Orlando EMA Standards of Care. Participating in Quality Improvement planning committee Other duties as assigned.
KNOWLEDGE, SKILLS, AND ABILITIES
Must have strong administrative, interviewing and communication skills to ensure an applicant has been appropriately determined eligible or ineligible. Ability to actively listen to others. Knowledge of counseling techniques and practices. Culturally and linguistically competent Working knowledge of targeted geographic locations. Basic computer skills Good organizational skills Knowledge of the dynamics, principles and practices of outreach to minority populations, Current knowledge of

disease transmission, counseling and testing requirements, prevention activities, program management and referral coordination. Bilingual ability a plus Must have a valid driver’s license and reliable transportation.
