This position will assist the department staff in Community Services, Meals on Wheels, and Weisman Delray Senior Center with the implementation of client assessments, and care plans for seniors receiving services through the agency. Responsibilities Manage the 701A and B Assessment tool and case narrative notes for the Aging in Place grant Conduct 6 month reviews with clients Utilizing intergy by adding a note with date of completion and alert as well as keeping and maintain spreadsheets of all clients that have had assessment and reporting up to date information to each department head Qualifications Minimum Bachelor’s Degree in Social Work Experience working with senior population Presents a commitment to quality in all job related activities. Effective verbal and written communication skills. Strong organizational skills. Accept direction from department staff members. Strong computer skills (Microsoft Office programs) Must maintain a good driving record and show proof of insurance annually Ability to drive personal car to complete in home assessments or assessments at Center when needed
