The Case Manager will work closely with the clinical team to identify individuals struggling to maintain engagement in services, and aid/support to address barriers. The Case Manager will collaborate with community partners to assist in providing referrals and easy access to outpatient services for the community. The Case Manager shall maintain a close working relationship with all members of the treatment team to ensure delivery to clients of comprehensive treatment in an efficient and expedient manner.
Required Education:
High School Diploma, GED, or equivalent.
Preferred:
Bachelors degree in Human Services or related field from an accredited college/university.
Required Experience:
At least 1-year professional experience in Substance Abuse field

Maintain accurate documentation of client interactions, progress, and outcomes in compliance with organizational policies. Experience Proven experience in crisis management and behavioral health settings is preferred. Familiarity with crisis intervention strategies and techniques. Knowledge of Medicare regulations and child welfare systems is a plus. Strong skills in motivational interviewing, addiction support, mentoring, and workforce development are highly desirable. Excellent communication skills, both verbal and written, with the ability to build rapport with diverse populations.
