Case Manager Healthy MD Palm Beach Gardens, FL 33410
Position Summary:
Our Case Manager identify, engage, and transport individuals in need to our clinics so they can receive HIV awareness and prevention education and to ensure they are provided STD and HIV testing, counseling, and referrals. This program provides risk reduction awareness and advocacy and a different approach to life through street outreach, intervention counseling, and HIV prevention education.
Essential Duties and Responsibilities:

Recruit participants for HIV/STD/Hep C testing, treatments, and counseling services. Engages street contacts in risk reduction discussions. Gives out

risk reduction information. Makes home visits and reaches out to persons/patients that present an adherence problem to treatment. Ensure data collection in a timely, accurate, and consistent manner. Distributes preventive materials including safe sex kits. Provide Outreach and Recruitment activities including but not limited to representing the program and the agency at community and committee meetings, providing educational workshops and social marketing activities. Deliver patient medication as needed Performs other duties and responsibilities as assigned
Computer Equipment & Software Skill Requirements:
Full knowledge of Windows Office Suite; particularly Microsoft Word and Excel Basic understanding of computers
