Job Description
Case Manager Caring Connection of South Fl West Palm Beach, FL Provide case management to intellectually disabled individuals. Your role would be advocating for the recipient and identifying, developing, coordinating, and accessing supports and services on the recipient’s behalf. This job will start at part time with an average of 15-20 hours weekly and can be increased to full time. On the job training and mentoring provided. Hours are flexible based on clients needs and you work from home. 2 years case management experience is mandatory. Experience with Intellectually disabled is preferred. Computer skills are mandatory for case notes and reports. Bachelor degree required. Must be able to pass a Level 2 background screening
