- 5 PM Hybrid role for Palm Beach County, FL-based candidates only 80% travel required for member visits Valid driver’s license required LTSS Service Care Manager Responsibilities Evaluate member needs, available resources, and develop service care plans Conduct assessments with members, caregivers, and providers for care plan development Monitor service delivery, coordinate referrals, and follow up with members and caregivers Ensure care plans are executed without gaps and identify functional deficiencies Collaborate with discharge planners, physicians, and healthcare teams for proper coordination Assist members with grievances and appeals as needed Maintain accurate documentation within the system within 24 hours of completion LTSS Service Care Manager Requirements Bachelor’s degree in a healthcare-related field (Psychology, Sociology, etc.
