Regional Case Manager 2.9 2.9 out of 5 stars Palm Beach County, FL This is a Salaried/Full-time Position; the Case Manager is an employee of Twin Oaks Juvenile Development, Inc. {DBA} “Twin Oaks Forensic Outpatient Services (TOFOS)” This position is accountable for the overall coordination and facilitation of the delivery of services to clients admitted to Twin Oaks Forensic Outpatient Services in a designated region. The employee’s office location is maintained within their primary residence. While this position is remote, the applicant MUST reside in or around

County. Monthly Face to face client meetings are mandatory. Position Responsibilities The TOFOS Case Manager will attend (in-person) All scheduled Twin Oaks Court hearings. The TOFOS Case Manager shall serve as the liaison between the Florida Juvenile Court personnel, the Florida Department of Children & Families (DCF), the Florida Agency for Persons with Disabilities (APD), the Florida Department of Juvenile Justice (DJJ), and Twin Oaks Independent Contractors to include Competency Trainers and Forensic Evaluators. The TOFOS Case Manager will conduct face to face contacts with each regionally assigned client at least once monthly. The client’s Parent and/or Guardian are required to be present and shall meet with the Case Manager to review the provision of services performed by Twin Oaks. The TOFOS Case Manager once a month shall assess the client’s prognosis in being restored to competency and the findings of this review shall be documented in the Case Management Monthly Assessment. The TOFOS Case Manager will perform a monthly review the C•ompetency R estoration S ervice P lan to determine the needs in the client being restored of competency. The Case Manager will provide assistance in the coordination of referrals to identified service needs. The TOFOS Case Manager shall observe •client’s environment both at home and at school during the monthly contact visits. The TOFOS Case Manager shall immediately investigate and follow-up verbally to the TOFOS Director within 24 Hours of any incidents involving clients, personnel, or program events. Written documentation shall be presented by the Case Manager within 48 hours on all reported incidents. The TOFOS Case Manager shall maintain a separate client record within the regional office for all regionally assigned case. This record shall include all records from intake through discharge. The retention of these records shall be in accordance to the TOJDC record retention policy. The TOFOS Case Manager shall be an advocate for the client to link and/or refer to services such as therapeutic, psychiatric, psychological, educational, vocational, or medical services and to assist in the coordination for services to other community providers when appropriate. The TOFOS Case Manager shall plan and arrange for the psychological evaluation of regionally assigned clients based upon the requirements of the TOFOS Program Administration; as specified in Florida Statute 985.19; the Florida Department of Children & Families, JITP Statewide Coordinator; or by order of the committing Court order; and at the recommendation of completion of training. The TOFOS Case Manager is responsible to provide all required {electronic and paper} documentation at Intake (w/in 10 days of admission), Monthly updates (every 30 days), Discharge (w/in 48 Hrs of dismissal by the Court). This documentation shall be provided as directed by the TOFOS Administrator. The TOFOS Case Manager is considered as an on-call position and thus personnel are to be available by cell phone after 5 p.m. and on weekends to address emergency situations ONLY involving clients and/or trainers. Travel Required.

Education requirements: Bachelor’s degree in related area of study. Master’s Degree Preferred Experience requirements: Two (2) years experience working with children or adolescents required. Bilingual Preferred.
