Job Summary:

CPSFL is seeking a dynamic Targeted Case Manager ( Wraparound Facilitator) – The Village. This position is specific to the Village Transitional Housing Program located in Downtown WPB. The Village program assists single adults that have substance use needs. A Wraparound Facilitator should be available to assist the adults with services – including a full range of traditional and non-traditional mental health services, health, education, and social services.

Qualifications:

A bachelor’s degree from an accredited university or college, with a major in counseling, social work, psychology, criminal justice, nursing, rehabilitation, special education, health education, or a related human services field. A minimum of one year of full time or equivalent experience working with adults with a serious emotional disturbance. Have a Florida Driver’s License and access to transportation. Bilingual a plus.

Compensation:

$43,888 annually.
